Temporary fulltime position until March 29, 2019. The successful candidate will gather information through referral source, care team and client in order to assess the needs for service. Develops and implements service plans within the program guidelines, contract expectations and policy. Provides referrals, support, guidance and problem solving with clients to address issues related to family functioning and challenges.
